To arrange a consultation with Dr Sarita Pawar please fill in the pre-consultation form to introduce yourself, including a brief medical history and reason for making the appointment. Please fill details to the best that you can, as this will be discussed further at the consultation.
Your email will be sent directly to Dr Sarita Pawar, and will not be stored on the web server. Any information provided by you will be handled with care according to GDPR guidance. You will then receive a Consultation pack, payment details and a date and time for your Consultation.
The initial consultation will be for 60 mins and involves a full assessment of your symptoms, and past medical/ surgical and treatment history. It will also be useful to have a copy of any recent blood tests. It is important to know your weight / BMI and BP. This procedure is followed to ensure safe prescribing of treatments. You may be required to have further tests or a referral to a specialist.
A follow up consultation at 6 weeks is routine to ensure that instructions have been understood and you are happy with the treatment started. Further follow ups can then be arranged at this consultation. If you so prefer with your consent the clinic will communicate with your GP for further prescriptions and reviews.
